Thank you for your interest in TA / I~AS adaptive sports and adaptive outdoor adventures programs and events.

We are pleased to be offering a safe and efficient contactless online registration process. These forms need to be completed annually. Should you have any questions, do not hesitate to email me: adaptive.guide@gmail.com.

Each participant must complete the following: (1) Registration Form, (2) Health Form, (3) MOVE UNITED / TA Waiver & Liability Release Form, (4) MOVE UNITED / TA Media Release Form (5) Essential Eligibility Criteria (EEC) Forms.

Please familiarize yourself with the following information to optimize your experience with us.
